AHPRA English test score changes came into effect on 23 April 2026, updating the minimum requirements for health practitioner registration across Australia.


If your test result is borderline, this update could determine whether you qualify for registration, or whether you need to retake your exam. Some scores went down. Some went up. And one test switched from letter grades to numbers entirely.


Here is what changed, what stayed the same, and what you should do next.


AHPRA English Requirements for Health Practitioner Registration in Australia


AHPRA (the Australian Health Practitioner Regulation Agency) regulates over 960,000 registered health practitioners across 16 professions in Australia — including doctors, nurses, midwives, physiotherapists, pharmacists, and psychologists. It works with 15 National Boards to set and enforce professional standards.


If you want to work as a registered health practitioner in Australia, AHPRA registration is mandatory. English proficiency is one of the non-negotiable requirements.


One thing that catches many internationally trained practitioners off guard:

your visa English requirement (set by the Department of Home Affairs) and your AHPRA English requirement are separate obligations. Meeting one does not automatically satisfy the other. The AHPRA standard is generally higher.


AHPRA English Test Score Changes

AHPRA aligned its minimum scores with updated score concordance research published in August 2025 and the Department of Home Affairs current migration standards. Score concordance research compares results across different English tests to confirm they measure the same level of proficiency.


When test providers update their scoring systems, the numeric thresholds shift, but the underlying English standard stays the same. These changes reflect that recalibration, not a raise or lowering of the bar.

Which Score Table Applies to You

The rule is simple, it comes down to when you sat the test:

  • Test taken on or before 22 April 2026 → old scores apply

  • Test taken on or after 23 April 2026 → new scores apply

  • Two sittings split across the date → first sitting assessed against old scores, second sitting assessed against new scores


AHPRA accepts results from one sitting, or a maximum of two sittings within a 12-month period. Results from different test providers cannot be combined. When combining two sittings, your component scores must meet the minimums across both results, and no individual score in either sitting can fall below the floor scores below.

Floor score = the lowest score AHPRA will accept in any single component across either sitting. Even if your combined result meets the minimum, a score below the floor in any component disqualifies that sitting.


Key Changes Worth Paying Attention To

PTE Academic — the AHPRA PTE requirement overall dropped from 66 to 63, which sounds easier. But the AHPRA PTE speaking score jumped from 66 to 76. If speaking is your weaker skill, this change works against you even though the overall is lower. Check your strengths before choosing this test.


TOEFL iBT — overall AHPRA TOEFL score dropped from 94 to 91. Most components also dropped slightly, but speaking increased from 23 to 24.


Cambridge C1 Advanced — now has differentiated component scores for the first time. Speaking (194) sits significantly higher than the other components. If Cambridge is your test, pay close attention to speaking preparation.


Cambridge C2 Proficiency — no change. If you hold a C2 result or are planning to sit C2, nothing has changed. All component scores and the overall remain identical to the previous standard. This is the only accepted test where every score stayed the same.


IELTS Academic — no change. The AHPRA IELTS requirement remains 7.0 overall with 7.0 in listening, reading and speaking, and 6.5 in writing. If IELTS is your strongest option, the goalposts have not moved.


OET — this is the biggest format change. The letter grade system (B, C+) no longer applies for tests taken from 23 April 2026. The AHPRA OET requirement is now expressed as numeric scores. If you hold an older OET result in letter grade format, it is assessed under the old standard, no conversion needed. If you hold an OET result in letter grades taken on or before 22 April 2026, it is assessed under the old standard, B for Listening, Reading and Speaking, C+ for Writing. No conversion to the numeric system is required.


What You Should Do Next

Most people read a policy update and do nothing with it. Here is how to actually use this:


You are planning to take a test — use the new score table above. Choose the test that suits your strengths given the updated component weightings. If your speaking is strong, IELTS is still a safe option with no changes. If you want flexibility, check PTE carefully — the overall is lower but speaking is now significantly harder.


You already have a test result — check the date first. If taken on or before 22 April 2026, old scores apply. Then confirm your result is still within the two-year validity window from your planned registration lodgement date.


Your result is close to expiring — do not wait. Test results are valid for two years before the date you lodge your registration application. That means if you plan to lodge in October 2026, your test must have been taken no earlier than October 2024. Two years goes faster than people expect, especially if registration is delayed by skills assessment or employer sponsorship timelines.


You are applying for both a visa and AHPRA registration — this is worth getting right before you book your test. The right score at the right level can potentially satisfy both requirements in one sitting, depending on your visa stream and occupation. Confirm your strategy with a registered migration agent first.


Test Rules and Retakes

Validity: English test results are valid for two years before the date you lodge your registration application. Exceptions apply if you have been continuously working as a registered health practitioner in a country where English was the main language of practice — or continuously enrolled in a Board-approved program of study — since your test date. Specific conditions apply; verify directly with AHPRA.


Two sittings: AHPRA accepts a maximum of two sittings within a 12-month period, from the same test provider.


IELTS One Skill Retake: AHPRA accepts the IELTS One Skill Retake, allowing you to re-sit one component within 60 days if you already meet the other three. This flexibility does not apply to other accepted tests.


At-home tests: Not accepted for any test. The OET computer-based test (at test centre only) has one remote-proctored element that AHPRA has specifically approved — this is the only exception.


TOEFL iBT: You must select "Taking TOEFL for Australia" when registering for your test. Without this, your result will not be valid for Australian registration purposes.

  • IELTS Academic — paper and computer at test centre only

  • OET (Occupational English Test) — paper or computer at test centre

  • PTE Academic — computer at test centre only

  • TOEFL iBT — computer at test centre only; must select "Taking TOEFL for Australia" at registration

  • Cambridge C1 Advanced or C2 Proficiency — paper and computer at test centre only

Medical practitioners also have two additional options: the New Zealand Registration Examination (NZREX) and the PLAB test.


Does This Affect Your Visa Application?

Your AHPRA English requirement and your visa English requirement are separate. Meeting one does not satisfy the other.


If you are a health practitioner applying for a 482 Skills in Demand visa or 186 Employer Nomination Scheme visa, your visa has its own English thresholds set by the Department of Home Affairs — generally lower than the AHPRA standard.


In some cases, holding a professional registration that required English proof can qualify you for an exemption from the visa English requirement, depending on your occupation and stream.

Frequently Asked Questions

Do I need to retake my English test if I am already registered with AHPRA?

No. Once you have demonstrated you meet the English language standard and are registered, you do not need to retake the test to remain registered. AHPRA may reassess English proficiency only if a concern is raised about your practice.


My AHPRA English test result was taken before 23 April 2026. Does it still count?

Yes, provided it is still within the two-year validity period from your planned registration lodgement date, and your scores meet the old minimums in Table 1 of AHPRA's transition arrangements policy.


Can I use the same English test result for both my visa and AHPRA registration?

Potentially. If your result meets the higher AHPRA score threshold and falls within the relevant timeframe for both requirements, it may satisfy both. Confirm this with a registered migration agent before booking your test — the answer depends on your specific visa stream and occupation.


I took TOEFL iBT at home. Will AHPRA accept it?

No. AHPRA does not accept at-home or remote-proctored test versions. You must sit at a test centre and select "Taking TOEFL for Australia" at the time of registration.


OET used to use B and C+ grades. What are the new numeric equivalents?

For tests taken from 23 April 2026, the minimums are: Listening 350, Reading 360, Writing 350, Speaking 360. AHPRA states the underlying proficiency level is equivalent — the scoring format changed, not the standard itself.
